Center for Integrated Health Solutions (CIHS) – The CIHS website contains a wealth of resources related to planning, implementing, and sustaining integrated health care. It’s a good starting place when looking for IHC information.
Standard Framework for Levels of Integrated Healthcare – This framework from CIHS provides a useful tool for assessing the extent of coordination and integration of care.
Agency for Healthcare Research and Quality (AHRQ) Academy for Integrating Behavioral Health and Primary Care “Lexicon for Behavioral Health and Primary Care Integration” – AHRQ’s “Lexicon for Behavioral Health and Primary Care Integration,” developed by expert consensus, provides language, definitions, and concepts for a shared understanding of what behavioral health and primary care integration is in practice.
Partners in Health: Mental Health, Primary Care, and Substance Use Interagency Collaboration Tool Kit (2nd edition, 2013) – This toolkit, produced by the Integrated Behavioral Health Project (IBHP), a California-based initiative, is an excellent resource on implementing and sustaining integrated health care partnerships. It contains a wealth of practical information and examples, such as sample MOUs, job descriptions, and various tools and forms. It is long (about 350 pages), but the table of contents is fully linked so that you may easily access information of interest. Although some of the information is specific to California, much of it applies regardless of location.
